Due to the increasing numbers in Unemployment Claim Fraud the Kansas Department of Labor has increased their security measures when filing for your unemployment. Kansas may now have to provide additional identity verification to receive their unemployment insurance benefits. According to the Kansas Department of Labor website, "...House Bill 2196 now allows claimants to verify their identity before a Kansas law enforcement officer. This form will be used by KDOL to establish the claimant's identity and, if the claimant is found eligible for UI benefits, process any outstanding claims."

The following website will allow you to download the form to bring to your local law enforcement agency and answer any other questions you may have: https://www.dol.ks.gov/law-enforcement-identity-verification or you may click here to print out the form.
